Things you will need for registration:​

• Grade Verification

• Proof of Residency (mortgage/rent or utility statement)

• Physical Form filled out by doctor - found HERE

• Health Insurance Card (or waiver)

• Pay Registration Fee (on our SHOP page)

**These items need to be turned in at the time of registration. You can not complete registration without these forms!**
